In order to obtain access to our extended profile Donor database, Family Source Consultants, LLC requires that Intended Parents agree to maintain confidentiality. Intended Parents agree that the information viewed will not be shared with any third party other than the spouse/partner or immediate family members and that username and password information will be kept confidential. Upon receipt of Intended Parents’ database inquiry form, Intended Parents will receive a password via e-mail within 24 hours.
