This
estimation does not include the cost of the IVF cycle and
medications; there are many variables that will determine this
amount depending on the clinic/RE chosen, the required
medications, and the amount of transfers necessary to achieve
pregnancy. If the Intended Parents have insurance that covers
infertility, however, IVF cycle and medication expenses will
likely be significantly lower than the amount estimated in the
fee schedule above.

Please note that the above fees are examples of what the Intended
Parents may expect. Fees are estimated and they will vary with every
Surrogate, as Family Source does not set the fee for the
Surrogate. Fees are subject to change without prior notice.

FSC is happy to offer basic profiles of Surrogates; however, to view
profiles in their entirety we require Intended Parents to submit a
personal/fertility information form. Once a Surrogate has been chosen,
FSC requires that the Intended Parents information packet is on file
before we contact the Surrogate. After the Surrogate reviews the
Intended Parents' profile and and agrees to go forward, the first
portion of the agency fee (50%) is due within three (3) days.
When the Surrogate is medically cleared, the final portion of the agency
fee (50%) is due within three (3) days.
Refund Policy:
Agency fees
are non-refundable. If the Intended Parent/s match with a Surrogate and
she decides not to go forward before the transfer has taken place—or if,
during medical or psychological screening it is determined there is a
problem that disqualifies her from becoming a Surrogate—Family Source
will match the Intended Parent/s with another Surrogate at no additional
charge.
